Formation

The SW&FO Formation Program is a proposed program for students who have committed a major offense. Its goal is to improve the individual by correcting faults and owned weaknesses or by strengthening good qualities since people are humans with inequities, shortcomings, and misdeeds in the process of growing up.

THE FOUR STAGES OF FORMATION PROGRAM

A. Monitoring and Updates Involved: DOs/GC
B. Seminar Involved: FPC/GO./ULFO
C. Assessment And Evaluation Involved: FPC/SW&FO Director
D. Presentation And Approval Involved: OSA “Dean”

A. MONITORING/UPDATES:

1. The Student Formators monitor:
a. if there will be any succeeding offense.
b. the observance of an individual’s sanction.
c. visitation made at the guidance office.

2. The GUIDANCE COUNSELOR shall:
a. take initial interview.
b. process the individual.
c. keep the records confidential.


B. SEMINAR:

1. GUIDANCE OFFICE
a. Structured-Learning-Experience
b.. Lectures
2. ULFO
- Presentation of St. La Salle’s value

C.ASSESSMENT and EVALUATION:

1.Collate all information
2.Scrutinize
3.Evaluate
4.Endorse to the SW&FO Director

D. PRESENTATION and APPROVAL

A special meeting to be held, composed of the:
1. Dean of Student Affairs/duly approved representative
2. SW&FO Director
3. Formation Program Committee


MECHANICS

a. The FORMATION PROGRAM COMMITTEE (FPC) is given a copy of the names of students who have committed major offenses, classified as graduating and non-graduating.
b. Information is disseminated regarding the implementation of the program.
c. Application form shall be available from the SW&FO clerk.
d. The Formation Program Committee shall handle screening of the participants.
e. Students who will respond positively to the invitation will have a meeting with the FPC.
f. The seminar shall be tentatively scheduled on January which shall be handled by the FPC.
g. With all the necessary requirements (monitoring and evaluation papers), the committee will eventually discern who passed the program.
h. Upon the recommendation of the SW&FO Director, the list of students who passed the said program will be presented to the Dean of Student Services.. Certificate of Completion shall be given to students who have completed the said program, which shall serve as a key in securing the Certificate of Good Moral.
